Summary
The WP4 series: system failure as the expression of inconsistent formation conditions. Technological structures, human factors, and institutional constraints each form according to their own internal logic and are only afterward combined into a unified system, producing "interaction without unity." Optimization at the operational level cannot change the conditions from which outcomes arise.
